• Pre-School and Kindergarten - Discovery Elementary School

• Pre-School Eligibility Requirements:

• Born October 2, 2008 thru October 1, 2009

• Kindergarten Eligibility Requirements:

• Born October 2, 2007 thru October 1, 2008

• Grades 1-2 Living on JB-MDL - Atlantis Elementary School

• Grades 1-2 Living in the Township - C.B. Lamb

• Grades 3-4 - C.B. Lamb

• Grades 5-6 - Upper Elementary School (UES)

REGISTRATION IS A TWO PART PROCESS

Step 1: Online Pre-Registration

• Print your confirmation page for your records

• Click Here To Print and Complete ALL District Registration Forms:

• Student Demographic Form

• Transportation Form - One per family, list all siblings

• Health History Questionnaire

• Health Information Release Form

• Physical Examination Form - Must be completed by a physician

• School Choice Option for JB-MDL Residents

• Home Language Survey

• The school secretary or clerk will call you to set up an on-site registration finalization meeting with in:

• 1 school day during the school year (September - June)

• 4-5 business days during the summer months (July - August)

Step 2: On-Site Registration Finalization:

• Bring the following to the on-site registration finalization:

• Completed district forms listed in Step 1

• Original Birth Certificate

• Proof of up-to-date immunizations

• Proof of Residency

• Examples of acceptable forms of proof residency include:

• Utility Bill

• Tax Bill

• Lease Agreement (For Base Housing Lease Agreement, pages 1 and 3 are sufficient)
